Remote Cardiology Patient Navigator

About the role

We are seeking a Remote Patient Care Coordinator, also known as a Cardiology Patient Navigator, to join a busy US‑based cardiology team. Working fully remote, you will support patient communication, scheduling, and care coordination for a high‑volume caseload of 150+ cardiology patients, primarily during US Central Time business hours.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows within EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions, coordinate next steps, and assist patients navigating cardiology pathways, including CHF management.
• Track testing schedules, remote monitoring data, and follow‑up appointments, ensuring continuity of care.
• Escalate clinical concerns to providers within scope (no diagnosing or prescribing).
• Maintain accurate clinical documentation, ensure HIPAA compliance, and support billing and insurance coordination as needed.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent).
• Minimum 3 years of experience in healthcare, telehealth, or medical office settings.
• Proven experience in care coordination or management, preferably in a remote or telephonic environment.
• Strong phone communication skills, organized under pressure, and comfortable handling a high‑volume workload.

Required skills

• Proficiency with EMR/EHR systems.
• Experience using remote patient monitoring platforms.
• Knowledge of HIPAA compliance standards.
